How to Obtain a Motor Vehicle Collision Report from Mississauga Police

You must have been directly involved in the collision to request a copy of the motor vehicle collision report.

Important Note: As of September 1, 2023, Peel Regional Police no longer release self-reported motor vehicle collision reports. If you attended a Collision Reporting Centre, please apply online through the Ministry of Transportation (MTO) for a copy of your police accident report.

Application Process for Law Firms & Insurance Companies

Requests for motor vehicle collision reports must be mailed on official company letterhead with details of the incident. Include:

  • Signed consent from your client authorizing the release
  • Company cheque covering the applicable fee:
    Occurrence Reports: $33.00
    Motor Vehicle Collision Reports: $40.00
    Notes/Statements: $151.00

Citizens may visit Peel Regional Police to request a police accident report. Bring the following:

  • Completed Form D
  • Two pieces of valid government-issued ID (one must include a photo; expired IDs are not accepted)

  • Driver’s licence
  • Passport
  • Firearms licence
  • Ontario Photo Card

Examples of Other ID:

  • Birth Certificate
  • Study/Work Permit
  • Immigration Documents

Please allow 15 business days for processing. Completed reports will be mailed or emailed.
