Individuals requiring a Motor Vehicle Collision Report can visit Service Ontario’s webpage for detailed instructions on ordering reports:
- Online: Order through Service Ontario’s online system.
- In Person: Visit any Service Ontario Centre.
- By Mail: Send your request to the Ministry of Transportation.
The fees are as follows:
- Certified Copy: $18.00
- Uncertified Copy: $12.00
Driver Copy: Information required under s.200(1)(c) of the Highway Traffic Act (commonly called the “driver copy”) can be obtained free of charge by contacting the responding officer directly via email.
For additional requests related to:
- Property damage reports
- Loss reports
- Witness statements
- Officer notes
You can apply online through the relevant police service portal.

You can apply for a Motor Vehicle Collision Report:
1. Online through the Service Ontario portal.
2. By Mail:
- Include:
- Two pieces of valid government-issued identification (one must include a photo)
- Payment via certified cheque, money order, or bank draft
