How to Request a Police Accident Report from North York Police Service

To request a police accident report in person, please visit the Customer Service Centre at Toronto Police Headquarters, located at 40 College Street, Toronto, ON. The Centre is open Monday through Friday from 7 a.m. to 4:30 p.m. Please note, requests for a police accident report are not accepted at Police Divisions.
Request forms for a police accident report are available at the Customer Service Counter or can be accessed online.
Required Documents for a Police Accident Report:
When visiting in person to obtain a police accident report, ensure you bring the following:
- Two (2) pieces of government-issued identification (at least one must include a photograph)
- Payment of the applicable fee: $67.80 for a Motor Vehicle Collision Report
Payment Methods:
- Visa, Mastercard, Amex, Interac (debit)
- Certified cheque or money order payable to Toronto Police Service
Note: Cash payments are not accepted at the Customer Service Centre.
To help locate your police accident report, please provide as much information as possible:
- Officer’s name and badge number
- Police accident report number
- Date of collision
- Location of collision (road name, street name, or intersecting streets)
- Names of all involved parties
- Vehicle plate numbers involved
By Mail
If you prefer, you can request your police accident report by mail. Follow these steps:
- Complete the official request form or write a letter specifying your request for a police accident report.
- Include the following:
- A photocopy of two pieces of government identification (one must include a photograph)
- Payment for the police accident report fee: $67.80 via a certified cheque or money order payable to Toronto Police Service.
- Provide the following information in your letter:
- Plate numbers of vehicles involved
- Names of involved parties
- Location of collision (road name, street number, or intersecting streets)
- Date of the collision/occurrence
- Officer’s name and badge number
- Police accident report number
